This is something I've poured so much care into, and I'm so glad it's finally ready to share with you.
If you've ever sat in a doctor's office and watched your mind go blank, or left an appointment thinking of everything you forgot to say, this is for you. The Rise Above Lyme Symptom Checklist is a six-page tool with more than 90 symptoms organized by body system, so you can check off what you've experienced, past or present, and walk in with the full picture.
What it is, and what it isn't. It's a pattern-tracking tool to review with a Lyme-literate provider. There's no score, and it isn't a diagnosis. You just check what applies and let your doctor see what you've been living with, in your own words.
What's on it
*A spot for your top three symptoms right now, so the most important things come first (appointments are short!)
*Symptom lists by body system: immune, muscles and joints, brain and nerves, eyes and ears, heart and lungs, mood and sleep, digestive, skin, and general health
*An extra section of patterns worth tracking, like symptoms that happen when you stand up, or that change depending on which building you're in
*A "worth mentioning" page and a notes space for anything that isn't listed
How to use it

2. Fill it out on your device or with a pen.
3. Bring it to your appointments, or send it to your provider ahead of time.
4. Re-mark it each month to see how things change as you treat.
